Principal Software Engineer, Clinical Decision Support

Our client, a value-based care healthcare company, is hiring a Principal Software Engineer to design, build and run the clinical decision support and API platform that puts timely clinical insights directly into provider workflows. You will own the technical foundation for CDS Hooks and REST APIs, working at the intersection of healthcare interoperability, API design, security and reliability, in a small, senior engineering team.

• Lead design and implementation of the CDS Hooks platform, including card orchestration and hook context handling.
• Own CDS Hooks API contracts in line with the SMART CDS Hooks specification.
• Lead technical integration with EMR systems such as Epic, NextGen, athenahealth and eClinicalWorks.
• Design and maintain REST and event-driven APIs exposing clinical data, quality metrics and care gaps to partners.
• Implement API security: OAuth 2.0, scope-based authorization, rate limiting and audit logging.
• Design real-time data pipelines with low latency and graceful failure modes.
• Set reliability SLAs and build monitoring, alerting and incident recovery.
• Ensure HIPAA and regulatory compliance, with access controls, encryption and audit trails for all clinical data access.
• Mentor engineers through design reviews and pairing.

• 8+ years of software engineering experience, including senior or principal-level architecture ownership.
• Deep experience designing public REST APIs, versioning and OpenAPI documentation.
• Healthcare interoperability experience: FHIR, HL7, CDS Hooks or EHR integrations.
• Hands-on API security: OAuth 2.0, authorization scopes, audit logging.
• Experience building reliable, low-latency data services in the cloud.
• Working knowledge of HIPAA and handling protected health information.

• Direct integration experience with Epic, athenahealth, NextGen or eClinicalWorks.
• Background in value-based care or population health.
